What you need to know before your first session:
Your appointment time is also your session start time. PLEASE PLAN TO ARRIVE 10-15 MINUTES EARLY so that we have time for a brief consultation. If you are late, your appointment has started without you and will still end on time. Please help me to give you the full session you are paying for by being early.
At the time of the telephone confirmation of your appointment, you will be asked to complete an intake form that corresponds to the type of service you have scheduled. We will not be able to see you if the form is not completed.The time it takes for you to complete the forms, along with the necessary consultation, could run into your session time. PLEASE COMPLETE THE APPROPRIATE INTAKE FORM IN ADVANCE OF YOUR SESSION and BRING IT TO THE APPOINTMENT WITH YOU. We will review the completed form when you arrive.
Unless you are receiving Swedish massage, you will remain fully clothed during your session, except for shoes and jewelry. Please wear pants that are not restrictive at your waist, not tight at the hip, knee, or ankle joints, and that do not pull on the skin of your legs. Short shorts and jeans are NOT a good option, but you will be able to change into appropriate, alternate clothing during the session, if necessary. Sweatpants or yoga pants are good options. Shirts should likewise not be overly restrictive at the shoulder, elbow, or wrist joints and preferably do not have excessive buttons or zippers. A t-shirt or tank top is always a good option.
Please also bring a pair of clean socks to wear during the session. If your feet tend to sweat, bring an extra pair of socks to change into during the session. If you are receiving a Swedish massage, you will undress to the level of your comfort and will be covered securely the whole time. Draping is the law. I will uncover only the area of your body that I am working on as it is being worked.
Personal hygiene is mutually respected on both the part of the client and the massage therapist. Your therapist is expected to be being punctual, well-groomed, free of odors, and wearing clean clothes. We ask that our clients demonstrate the same respect by arriving on time and clean. As a courtesy to the therapist and other clients, please refrain from using cologne, perfumes, or essential oils before arriving to your session. Should either party fail to uphold their hygiene responsibilities, services for that session will be postponed.
Please refrain from eating at least 90 minutes prior to your session and consume a light meal at that time.
The office is located on the northwest corner of Oakland Avenue and Glendale Avenue at 4601 N Oakland Avenue, Shorewood, WI 53211. Look for the large “Dr. Zhou’s Acupuncture & Wellness Center” sign. Upon arrival, enter the building parking lot from Glendale Avenue. If there is no space in the lot, please park on Glendale Avenue or Oakland Avenue and enter the building through the main door that faces east on Oakland Avenue.
If the main entrance door is locked, please wait patiently and I will be down to open it for your within 15 minutes of your appointment time. If the door is NOT locked, enter the building and take the elevator to the second floor. Turn right when you exit the elevator and walk through the door into the hallway. Make an immediate left turn and enter Suite 221. If the door to Suite 221 is CLOSED, please have a seat in the large second floor lobby right outside the elevator.
Cash is the preferred method of payment due to the various fees and delays associated with credit card processing, however I do require a credit card on file (see cancellation policy, below). Personal checks are NOT accepted. As a professional health care practitioner, tips for full priced sessions are appreciated but not expected. My pricing is aligned with the industry standard for a professional massage therapist with my level of training and experience. From time to time I offer a Groupon, Youpon (internally generated coupon), or offer a special through the newsletter I send to existing clients. Other than these offers, discounted services are generally not available, except as explained below.
Packaged options of multiple sessions will be offered at the end of your first session, after we have had an opportunity to work together and to determine if we are a good fit. If we conclude that your situation will benefit from multiple or ongoing therapy, we can discuss a prepaid package at that time.
A note on Prepaid Sessions: All payments including prepaid packages, sessions, or gift certificates are non-refundable and must be used within FIVE YEARS of purchase. After one year from purchase, an $20 annual fee will be deducted from unused balances until the balance has been depleted or until the card expires, whichever is sooner. Exceptions to this refunds and extensions policy are made at the sole discretion of Chokkan Shiatsu & Wellness Services. Prepaid, unexpired sessions are transferrable.
I have reserved your appointment time JUST for you. By scheduling an appointment with me, YOU AGREE TO GIVE AT LEAST 24 HOURS-NOTICE FORCANCELLATIONS/RESCHEDULING.
If a true emergency presents itself that conflicts with your appointment, I ask that you notify me immediately through any medium necessary. You can email me at any time of day or night, or make a phone call and leave a voice message any time during normal waking hours, but it is your responsibility to ensure you have received a reply from me to confirm cancellation BEFORE your missed appointment time. I also accept text messages at the (414) 502-7336 .
If you have circumstances that you feel deserve special consideration, please let me know as soon as possible so that I can decide whether to make an exception to this policy.
Make sure the email you received upon booking your session is accurate and that the date and time of your appointment agrees with our stated arrangement. Please notify me as soon as possible by text at (414) 502-7336 if you believe there is a discrepancy. The sooner you notify me, the sooner any necessary corrections can be made, and your appointment can be rebooked. It is recommended that you respond to the text/email appointment notice within 48 hours of your appointment date/time in order to confirm your appointment or let me know immediately if you need to change or cancel.
IF YOU HAVE ANY OF THE FOLLOWING CONDITIONS, PLEASE OBTAIN A NOTE FROM YOUR DOCTOR PRIOR TO YOUR APPOINTMENT AND BRING IT WITH YOU TO YOUR APPOINTMENT. Arriving for your session with any of the below conditions and without the fully completed Physician Permission form will require that we reschedule to a time when you are able to receive massage and you will be charged full price for your massage hour. I WILL BE UNABLE TO SEE YOU IF YOU ARE CURRENTLY EXPERIENCING ANY OF THE FOLLOWING:
Practitioners in private practice, such as massage therapists, have the right of refusal. That is, massage therapists can refuse to work with any existing or potential client. Furthermore, massage therapists have the right to end any session for any reason. Chokkan Shiatsu & Wellness Services has a zero-tolerance policy. The following are examples of behaviors that WILL result in immediate termination of your session and permanent banning from future sessions:
Arriving to appointments while under the influence of drugs or alcohol WILL result in immediate termination of your session and MAY result in permanent banning from future sessions.
In order to provide your service without distraction or interruption, please do not bring anyone to your scheduled appointment. Please do not bring children to appointments unless they are receiving treatment. Children and visitors must be accompanied in the building, so it will not be possible to proceed with your session if you have unattended guests waiting for you.
Please notify me BEFORE your session, if you develop any new health conditions or symptoms, no matter how minor. Arriving for your session with any of the above conditions will require that we reschedule to a time when you are able to receive massage and you will be charged full price for your massage hour.
